Traffic Solutions Return Material Authorization (RMA) Form


Instructions for returning cameras for repair:

  1. Complete and e-mail the RMA-form (link provided to Word document below) to rk@jai.com (Rubirose Kosuma - camera repair department)
  2. JAI will email you an RMA number.
  3. Return the product with a label containing your assigned RMA number to the JAI service/repair facility in San Jose:

    JAI, Inc.
    Attn: Camera Repair Department (RMA# XXXXX)
    625 River Oaks Parkway
    San Jose, CA. 95134
    USA
  4. After repair, JAI will return the product to the “ship to address” you’ve provided in the Word document.

Please note: JAI does not accept repair returns without a completed RMA form and a valid RMA number.

Warranty Repairs
The RMA number must appear on the shipping label and all shipping documents related to the return. The product must be shipped CIP (Carriage and Insurance Paid) to JAI at the address shown above. For terms and conditions of warranty products, please refer to you warranty card enclosed with each shipment.

Non-Warranty Repairs Flat Rate
RMA number must be provided by JAI prior to product shipping.  RMA number is to appear on shipping label and all paperwork related to the return. Progressive Scan – B/W and Color repair charge = $315.00 */** Progressive Scan – B/W and Color repair charge including exchange of CCD = $722.00*

Problem reported but not found = $315.00 USD

Repair includes re-calibration of the camera.

*Note:  There may be exceptions, such as cameras with standard or custom options.  In these situations, JAI will contact customers for repair approval in advance of work commencing.

**Exclusive of the flat rate policy is:  CCD replacement, intensifier tube replacement/repair, fiber coupling, and user damage or abuse.
